All forms are due by May 15th.*  You can either complete the forms and mail them to Liana Voss, or you can scan and e-mail them to  Scanning is the preferred method.


*The MHSAA physical form is not due on 5.15.20.  Currently there is an order in this state prohibiting non-essential medical procedures, including physicals.  Physicals will be due on the first day of band camp on July 29th.    


Payments are also going to be different this year.  To help alleviate the stress of many of our parents being out of work, we are breaking up the payments a little differently. 


Here is the breakdown in due dates:


May 15th - Any uniform costs are due at this time.  This is because the order is placed the following week for summer uniforms, gloves, etc.  Please check your child’s old summer uniform to see if they need anything new because they grew. 


June 1st - 1st half of payment due.  For most families, this will be $70


July 1st - 2nd half of payment due.  For most families, this will be $70


Payments Options: 

  1. Checks can be sent to Liana Voss.  Address is on the Application Form

  2. Credit and Debit card payments can be made Online through Cheddar UP.  Click on the link to bring you directly to the Cousino High School Band Program page to pay by credit or debit card.
